Referral Process

- Initial Consultation Upon receiving the referral, our team will reach out to the patient to schedule an initial consultation, either in-person or via telehealth.
- Collaborative Care After the initial assessment, we will send a detailed treatment plan and progress updates to you to ensure the best care for your patient.
- Follow-Up We offer flexible follow-up appointments based on the patient’s needs and ensure communication with you throughout the treatment process.
